Hepatic angiomyolipoma presenting with chronic epigastric pain: A case report

چکیده
Introduction: Angiomyolipomas primarily arising from the liver are rare. Hepatic angiomyolipoma (HAML) is a rare tumor characterized by the presence of blood vessels, muscle tissue and adipose tissue with only about 300 reported cases to date. This is the first case reported in literature of a Filipino patient. Case Report: A 49-year-old Filipino female presented with a two-year history of recurrent epigastric pain. Abdominal magnetic resonance imaging revealed a fat-containing lesion in the left liver lobe. Serial follow-up revealed progressive enlargement on imaging studies. Serologic tests were negative for malignancy or infection. The patient underwent left hepatic lateral segmentectomy which revealed a 7.5x7 cm friable mass involving hepatic segments II and III. Histopathology showed mature fat cells with a few thick-walled blood vessels and spindled smooth muscle cells with no atypia. Homatropine methylbromide-45 test showed strong and diffused staining confirming angiomyolipoma. Conclusion: Clinical evaluation and a comprehensive analysis of ultrasonography, Anthony Relucio Perez1, Clarence Yacapin2 Affiliations: 1MD, MHA, Vice Chair for Training, Department of Surgery, UP College of Medicine and Philippine General Hospital, Section Chief, Hepatobiliary and Pancreatic Surgery Asian Hospital and Medical Center; 2MD, Clinical Fellow Section of Minimally Invasive Surgery, Department of Surgery Asian Hospital and Medical Center. Symptoms, malignant potential and inability to distinguish from liver malignancies may indicate surgical resection as the best treatment option.

Hepatic infarction and acute pancreatitis: a case report and review of the literature
Hepatic infarction is rare due to the unique dual hepatic blood supply from the hepatic artery and the portal vein. Herein, we report a case of hepatic infarction that occurred as a complication of acute pancreatitis. The patient was a 58-year-old male with past medical history of chronic alcoholism, who presented with epigastric abdominal pain, nausea, and vomiting. Hepatic infarction was diag...
متن کاملA case of ruptured hepatic angiomyolipoma in a young male
A 31-year-old male visited a local hospital due to sudden-onset severe abdominal pain. Abdominal computed tomography revealed a solid cystic mass with a size of approximately 12 cm and exhibiting both hemorrhage and fluid collection in the pelvic cavity. Emergency angiography and embolization were performed, and a large hepatic tumor was subsequently surgically resected. The tumor cells stained...

INTRODUCTION A hepatic angiomyolipoma is a rare benign tumor of the liver composed of a mixture of smooth muscle cells, blood vessels and a variable amount of adipose tissue. Differentiating them from malignant liver tumors can often be very difficult. CASE PRESENTATION We report the case of a 43-year-old Caucasian man presenting with a large liver mass in the right lobe. The results of magne...
متن کاملRuptured hepatic artery aneurysm presenting as abdominal pain: a case report
Aneurysms of the hepatic artery are rare. This patient presented to the emergency department with severe epigastric pain and subsequently became haemodynamically unstable. Plain abdominal radiograph showed a ring lesion in the right upper quadrant, ultrasound scan demonstrated a mass with arterial blood flow, and computed tomography revealed a left hepatic artery aneurysm. At surgery, the ruptu...
متن کاملIsolated Hepatic Tuberculoma – A Case Report
Isolated hepatic tuberculoma is localized hepatic tuberculosis (TB) without bile duct involvement, which presents as solitary or multiple nodular mass lesion of liver mimicking a neoplastic lesion in radiological evaluation. Clinical presentation and biochemical tests for liver functions show non-specific abnormality, which is not helpful for diagnosis. As the treatment, modality of isolated he...
